To redeem this offer, just fill out this mail in rebate form after you purchase a Life Scents product. Then, attach the original dated receipt with the UPC code handwritten on it and mail everything to the address listed on the form.

In about 6 to 8 weeks, you'll receive a cheque in the mail for up to $15.95, which will cover the cost of the product and taxes.

Each individual, family, or household is limited to one rebate.

Only purchases made between August 10 and October 25 will be eligible for this offer. The rebate form must be received on or before November 25.
